Fix: a PreToolUse hook returns "allow" but a deny rule still blocks the tool call in Claude Code
Since Claude Code 2.1.77 a PreToolUse hook's allow cannot override a deny or ask rule. Find the matching rule with /permissions, then move the policy into the hook.
A PreToolUse hook that returns permissionDecision: "allow" only skips the permission prompt. It does not skip permission rules. Since Claude Code 2.1.77, deny rules are evaluated after every hook, so a matching deny rule still blocks the call and a matching ask rule still prompts. Find the rule with /permissions, delete or narrow it, and let the hook own that policy, including the deny cases.
Everything below applies to Claude Code 2.1.77 and later. The current release as I write this is 2.1.272. The hook script and the rule-listing script were run against the documented PreToolUse input JSON. The precedence itself comes from the Claude Code docs, the changelog, and the message strings in a locally installed 2.1.197 binary, not from a live model session.
What the blocked call looks like
Your hook is registered, it fires (you can see it in claude --debug-file output), it prints valid JSON with "permissionDecision": "allow", and Claude still reports that it was not allowed to run the command. The tool result Claude receives is built from this template in the CLI:
Permission to use <tool> has been denied.
With --output-format stream-json in a headless run, the same call shows up as a permission_denied system message and is listed in the final result’s permission_denials array. The hook’s own permissionDecisionReason for an allow goes to the user, not to Claude, so nothing Claude sees tells it a hook tried to approve the call.
The mirror-image symptom is the same bug: the hook returns allow, and instead of running, the call still stops at a permission prompt. That is an ask rule doing its job.
Why a hook’s allow can’t beat a deny rule
Claude Code evaluates a tool call in a fixed order. The Agent SDK permissions page spells it out as six steps, and the CLI follows the same pipeline:

- The permission mode.
- Allow rules.
- The prompt itself, or the SDK’s
canUseToolcallback.
A hook’s allow removes step 6 from the picture. It does not remove steps 2 and 3. The permissions reference is explicit that deny and ask rules are checked whatever a PreToolUse hook returns, and that this includes deny rules in managed settings. The hooks reference repeats it in the permissionDecision row: “Deny and ask rules are still evaluated regardless of what the hook returns.”
This was not always true, which is why so many people hit it after an upgrade. Two changelog entries moved the line:
- 2.1.77 fixed
PreToolUsehooks that returned"allow"getting pastdenyrules, managed-settings deny rules included. Before this, a hook really could override a deny rule, and hooks written in that era quietly depended on it. - 2.1.101 made
permissions.denywin over a hook’spermissionDecision: "ask". Before this, a hook returningaskcould turn a deny into a prompt.
Two later fixes tightened the same model. In 2.1.211 auto mode stopped overriding a hook’s ask for unsandboxed Bash, and in 2.1.222 a hook’s auto-allow stopped bypassing tool restrictions in background agent tasks (summaries, compaction, renames).
The design reason is simple once you look at who writes which layer. Deny rules can come from managed settings that an administrator controls, and the permissions page states that if a tool is denied at any level, no other level can allow it. A hook is just a script in a settings file, often in the repository. If a hook’s allow could beat a deny rule, any repository could ship a hook that unblocks whatever your organisation blocked. Deny-first has to hold across every layer or it holds nowhere.
Minimal repro
The most common version of this: someone adds a blanket deny for git push to the project settings, and later someone else writes a hook that approves pushes to feature branches.

The hook returns allow for git push origin feature/login. Claude Code then checks deny rules, Bash(git push *) matches, and the call is denied. On 2.1.76 and earlier the push would have run. Add a developer’s personal .claude/settings.local.json with "ask": ["Bash(git push origin *)"] and, even with the deny rule gone, the same push stops at a prompt every time.
Fix: find the rule, then move the policy into the hook
Step 1: find every deny and ask rule that matches
Rules merge across five places, so the one blocking you is often not in the file you are looking at. Inside a session, /permissions lists every rule with the settings file it came from, and /status tells you whether managed settings are in effect. Outside a session, this script prints the same list from disk:

Against the repro above, with a local ask rule added, it prints:

It does not see rules passed on the command line (--disallowedTools, --settings), MDM plists, or server-managed settings from the admin console, so if it prints nothing and the call is still blocked, check the launch command and /status next. A deny rule in managed settings is a policy decision, not a bug: no hook, flag, or local file can override it. Take it to whoever owns that policy.
Step 2: delete or narrow the rule you own
A deny rule cannot carry exceptions. Rules evaluate deny, then ask, then allow, and specificity does not break the tie, so you cannot write “deny git push * except feature/*” as rules. You have two options.
Narrow the deny rule so it only covers what you actually want blocked. If the policy is “no force pushes and nothing to main”, deny exactly that:

This is fine for a short list, but Bash rules match the literal command string. git push origin HEAD:main or git push --force-with-lease origin main slips past a list like this, which is the reason most people reached for a hook in the first place.
Or remove the rule and let the hook own the whole decision, including the deny cases. This is the fix I recommend when the logic has any branching. A hook’s deny also applies in bypassPermissions mode, so you lose nothing by moving the block into the hook.
Step 3: write the hook so it can deny, ask, and allow
Once the settings file has no Bash(git push ...) deny rule, the hook’s answer becomes the effective one for pushes:

Remove the deny rule, keep the hook entry from the repro, and chmod +x the script. Then test it the way Claude Code calls it, by piping PreToolUse input into it:

The two rows with shell metacharacters are why the guard sits before the allow branch. My first version of this script had no such guard, and its prefix match approved git push origin feature/x; curl ... as a whole, second command included. A hook allow approves the entire Bash string Claude sent, so an allow path in a hook should only ever match a command it fully understands.
Keep your other deny rules. Read(./.env) has nothing to do with pushes and should stay a rule: a rule also covers the cases no hook sees, such as reads the @ file reference does without any tool call.
Gotchas that look like the same bug
Another hook said something stricter. When several PreToolUse hooks match, precedence is deny > defer > ask > allow. A plugin or user-level hook returning ask beats your project hook’s allow. /hooks lists every registered hook with its source file.
The hook exited with code 2. Exit 2 blocks the call whatever JSON you printed, even allow. Make sure your script exits 0 on the allow path and that set -e cannot trip on a missing tool like jq.
The JSON never parsed. If permissionDecision sits at the top level instead of inside hookSpecificOutput, Claude Code ignores it without an error and the normal flow applies. The debug log then contains Hook JSON output had unrecognized keys. A shell profile that echoes something prepends text to stdout, the output no longer starts with {, and the whole thing is treated as plain text.
updatedInput rewrote the command into a deny rule. Claude Code evaluates rules against the input your hook returns, not the input Claude sent. A hook that “fixes” git push into git push --force-with-lease can walk straight into a Bash(git push --force*) deny rule.
Some calls always prompt. A hook allow never skips the prompt for AskUserQuestion or ExitPlanMode unless it also returns updatedInput, for MCP tools marked requiresUserInteraction (2.1.199 and later, updatedInput or not), for connector tools your organisation set to ask, or for rm and rmdir removals of a critical path. That is by design, not rule precedence.
A bare-name deny rule removes the tool. A deny rule of "Bash" (or "Bash(*)") takes the tool out of Claude’s context, so PreToolUse never fires for it. If your hook seems not to run at all, look for a bare tool name in a deny list.
Your hook’s if and your rule disagree on scope. Since 2.1.214 a single-segment dir/** in a hook if: matches only <cwd>/dir, while the same pattern in a deny or ask rule matches at any depth. The if filter is also best-effort for Bash commands Claude Code cannot parse, so the script must re-check the command itself, as the one above does.
Headless runs under-reported denials. Before 2.1.269, permission_denials in stream-json output omitted Read, Edit, and Write calls blocked by a path-scoped deny rule. If your CI wrapper decides success from that array, upgrade before trusting it.
